📝 Your Core Responsibilities:
- Conduct comprehensive in-home physical or occupational therapy assessments
- Develop and implement personalized care plans focused on mobility, safety, and activities of daily living (ADLs)
- Provide hands-on therapeutic interventions and progress monitoring
- Educate clients, families, and caregivers on safe transfer techniques and home modifications
- Maintain clear, reliable communication with families and our clinical coordination team
✔️ What We Are Looking For:
- Active Massachusetts Physical Therapist (PT) or Occupational Therapist (OT) license (Required)
- A compassionate, professional, and dependable clinical work ethic
- Experience in home health, rehabilitative centers, or assisted living environments is preferred
- Strong communication, paperwork management, and teamwork skills
- Reliable transportation to attend scheduled client visits on time
